Endometriosis

Endometriosis-Symptoms, And Treatment:

10 out of every 100 women in the reproductive age suffer due to endometriosis.

Endometriosis is a condition in which the tissue lining the uteral wall grows outside the uterus as well. Typically, it grows outside the uterus on fallopian tubes, ovaries, and around the uterus.

In rare cases, it may even be found in other body parts.

You may experience pelvic pain, painful bowel, and heavy bleeding during the menstrual cycle.

If you have any of the above symptoms, you should visit a gynecologist without delay and neglect.

Only an experienced gynecologist will be able to diagnose this problem in the early stages.

Dr. Hrishikesh Pai is one of the top gynecologists in Mumbai who can successfully treat patients for endometriosis in time.

Dr. Pai has founded many IVF centers in major nodes of India like Mumbai, Delhi, and Gurgaon.

He founded Bloom IVF, Budget Fertility Centre, and Babies and Us.

Now, let us get to know more about endometriosis.

Endometriosis1

What is Endometriosis?

As explained above, the tissue of the endometrial lining grows outside of the uterus, on the fallopian tubes, ovaries, and around the uterus. In some rare cases, it will be found in other body parts.

You need to know that when the endometrial tissue is found outside of the uterus, it continues to behave as it does in the uterus.

It will break down and shed. But there is no place it can get disposed. It continues to stay in the body.

This causes the broken endometrial tissue to harden around the organ resulting in fibrosis.

If it is around the ovaries, it results in cysts called endometriomas.

What are the symptoms of endometriosis?

Endometriosis is a severe condition. It is painful during

  •       menstrual cycle,
  •       urination,
  •       bowel movement and
  •       during intercourse.

So generally speaking, any activity related to the pelvic organs is a painful affair if you have endometriosis.

It may cause infertility.

Your severity of pain is not an indicator of the severity of endometriosis.

If you have been experiencing the symptoms mentioned above, please schedule a consultation with  Dr. Hrishikesh Pai at Budget Fertility Clinic for a detailed, accurate and timely diagnosis of the cause of your symptoms.

Endometriosis2

What are the causes of endometriosis?

Although the exact cause is unknown, some probable causes are listed below.

Menstrual retrograde:

Menstrual blood, including endometrial cells, flows back through the fallopian tubes and into the pelvic cavity instead of out of the body during retrograde menstruation. These endometrial cells adhere to the pelvic walls and the surfaces of pelvic organs, where they proliferate, thicken and bleed during each menstrual cycle.

Transformation of the peritoneal cells:

It is a theory, not confirmed, called the “induction Theory.” Here the peritoneal cells lining the abdominal wall transform into endometrial-like cells. It may be a result of the hormones of the immune factors.

Transformation of the embryonic cells:

Another theory is that estrogen transforms the newly forming cells, the cells in the embryonic stage, into endometrial-like cells during puberty.

Transported endometrial cells:

The endometrial cells may travel through the blood vessels or the lymphatic system to other body parts.

Immunity disorder:

The immune system may be unable to recognize the presence of the endometrial-like cells in other parts of the body and destroy it.

What are the long-term effects of endometriosis?

Endometriosis may result in

Infertility

Early puberty

Late menopause

The short duration between cycles

Longer menstrual cycle

Heavy bleeding

Having high estrogen levels in your body for a very long time.

Cancer

Diagnosing endometriosis:

Endometriosis can be diagnosed with imaging technologies and also biopsy.

Some of the methods of diagnosing the disorder are:

MRI

Laparoscopic imaging and biopsy: While your surgeon takes to laparoscopy for visualization, in cases when lesions are not visible on the camera, the doctor may take a sample for biopsy. It is also possible to surgically remove the endometriosis at the same time.

Ultrasound: If it has been clinically established that you are suffering from endometriosis, the pelvic region ultrasound will help determine the location and size of the nodules.

It reduces the number of incisions your gynecologists will have to make to visualize and remove the endometria.

You can get a consultation at Budget fertility clinic in Mumbai and get your tests scheduled to determine the presence of endometrioma with Dr. Hrishikesh Pai.

Treatment and management of endometriosis:

There is no specific way to treat endometriosis since the cause is unknown.

The only thing possible for a gynecologist to do is to help you control your symptoms.

The most common practice is to put a patient on oral contraceptive pills.

Other than that, your gynecologist may give you pain killers to manage your pain.

Intrauterine devices for controlling the progesterone-releasing implants may also help prevent the production of endometrial cells. Gonadotropin-releasing hormone agonist may help a woman get pregnant.

Another way to keep the endometriosis under check is to remove the endometrial cells surgically from the affected area.

One of the most important part of managing your endometriosis is to remain physically active. Regular exercise prevents endometrial build up.

Even after the surgeon attempts to bring your normal pelvic anatomy by extracting the endometrioma, the chances of recurrence are high. Its 21.5% in 2 years and 40-50 % in 5 years.

What are your chances of conceiving?

Natural conception is difficult since you will be on oral contraceptive pills to manage endometriosis.

Your surgeon may, however, plan an IVF cycle post-surgically extracting the endometrioma.

But for this, your body has to undergo either a short pre-treatment to improve your fertility chances or long-term GnRH treatment, which has a higher success rate in IVF.

Dr. Hrishikesh Pai has a very high success rate in Assisted Reproductive Technology and is very successful at IVF pregnancies.

 Conclusion:

We conclude that there is no fixed treatment plan for endometriosis since the cause is unknown.

All that is possible in managing the symptoms since that is what is known.

Pregnancy, though difficult to achieve, could be successful through IVF.
